Integrated Pest Control - PEST EVIDENCE REPORT

Please fill in a report form when any of the following are found in a collection area: dead or live insects, insect cases, frass, droppings, loose fibres, evidence of other pests
Take sample of pest evidence in phial, mark it with the departmental sample number
Please forward report and sample to the Relocation Office as soon as possible
If deemed necessary a photo will be taken
Appropriate treatment will be arranged by the Relocation Office
