White paper pad with black print. 'PRODUCT/ WEEK NO./ NO/ CARTONS/ WEIGHT/ DATE/ Signature'. After each word there is a line of black ink on which to record the required information. There is a black square box in the left had corner of every page with a number above it. The number is the same on every page, '8979'. There is also a number written in red ink in the right hand corner which is different on every second page. The first of these numbers is 25501 and the last 5600. There is a thick piece of cardboard on the back of the notepad and glue along the top.
