Yellow paper pad with black print, a thick piece of cardboard at the back and glue along the top. The print reads 'PRODUCT/ WEEK No./ No. CARTONS/ WEIGHT/ DATE/ Signature.' After each word there is a line of lack ink on which to record the information. There is a black square box in the left hand corner of every page with a number above it in very small print. The number is the same on every page, '8979'. There is alos a number written in red ink in the right hand corner of every page which goes up one number every second page. The first of these numbers is 25501 andthe last 25600.
