Green paper pad with black and red ink, a cardboard back and ink along the top. The words 'PRODUCT/ WEEK No./ No. CARTONS/ WEIGHT/ DATE/ Signature' are printed on every page with a line of black ink after each to record the required information. There is a black box in the left hand corner of every page with a number above it written in very small print. The number, '8979' is the same on every page. There is also a number written in red ink in the right hand corner of every page which goes up one number every second page. The first of these numbers is '25002' and the last is '25100'.
