CARD, LONG SERVICE LEAVE

Thin plastic card about the same size as an automatic teller card. The front of the card is yellow, and the back white. The following information is printed in white on the front : "CONSTRUCTION INDUSTRY / LONG SERVICE LEAVE PAYMENTS BOARD / WESTERN AUSTRALIA / REGISTRATION CARD". The lower half of the front has the headings printed in white and the information has been added later in black. "REGISTRATION / NUMBER 7013284 / YEAR / OF EXPIRY 1990 / NAME RF PEET". On the back is the following information "REGISTRATION NUMBER / THE REGISTRATION NUMBER ON THIS / CARD SHOULD BE QUOTED IN ANY / CORRESPONDENCE WITH THE BOARD / CHANGE OF ADDRESS/ TO ENSURE YOU RECEIVE YOUR ANNUAL / NOTICE OF SERVICE CREDITS PLEASE / ENSURE YOU NOTIFY THE BOARD IF / YOU CHANGE YOUR ADDRESS." In the bottom right hand corner is "CPE Data Card". The card belonged to Ross Peet a plumber who works in Perth or up north, and lives in Margaret River when he has time. He has worked on the Princess of Wales wing at Fremantle Hospital, the Merlin Hotel and the R & I tower behind the Palace Hotel.
