Small square wood veneer block with metal plate featuring logo. The logo has a circle and inside this are the words 'VIVE' and 'VALEQUE' on the bottom periphery and flanking a shield-like shape with the letters 'F' and 'H' at centre. At the top of the shield and inside the circle is a maltese cross. Written in blue ink on one side 'FREMANTLE/ HOSP'. Handwritten thick pen mark on bottom of base.
