Former Kalgoorlie Mining Associates property. a. Box has metal handles at either end, leather handle in front. Lid reads "H-H Inhalator / SAFETY - EMERGENCY - SERVICES / MINE SAFETY APPLIANCES / PITTSBURGH PA." There is a small metal plaque which reads "Mine Safety Appliances Co / H-H Inhalator / Type K Serial no 5138 / Patent no 1940, 135". Seems very similar in mechanics to H89.764. There is room for two gas bottles, only one is in place at present. An instruction card is attached by drawing pins to inner lid. b. There is a treatment report booklet produced by the company - one page of the booklet is completed in pencil: "L. OLLSEN / BULLER ST, KALGOORLIE / 200 LEVEL / SUFFERING AFFECTS [sic] DINAMITE [sic] FUMES / ... BOULDER PERSEVERANCE LTD / FIMISTON W.AUST". Reverse of form is pre-printed postcard addressed to: "Yandel Henderson, PhD / Laboratory of Applied Physiology, / Yale University,/ New Haven, Conn." c. Form with specification of H-H Inhalator, pencilled in and completed. Inspected by E. Bailey, Feb 3 1939.
