Square plaque with slight points at top centre and bottom centre. Raw pewter finish with raised rim around outside edge. Slight chevron shape at top with text '2 YEARS/ NO LOST TIME INJURY'. Second larger chevron shape below, line detail border around edge, text 'TRANSPORT' and 'TOM PRICE' horizontally across top and bottom and 'ESTD/1982' running vertically on left and right sides. Circle shape in centre divided into thirds with red enamel centre and Hamersley Iron logo with letters 'H.I'. Tree motif in top left third, bus motif in bottom third and loader truck motif in top right third. Mine landscape motif in background to left and right, with lined hill motif around circle. Loader truck to left and dumper truck to the right. Reverse: raw pewter finish rim, green felt cover.
