W.A. shaped badge with nickel rim and reverse. Light yellow enamel background, becomes lighter at edges. Red enamel circle in top right quarter with two black enamel horizontal lines and text 'H.I'. Green enamel cross in bottom left quarter with text ' HEALTH SAFETY' running vertically and 'OCCUPATIONAL' written horizontally. Reverse: Nickel plate finish, 'SHERIDAN/ PERTH' running vertically. Brooch fixing with no pin or catch attached.
