Has a linear red border, crossing at each corner and printed in purple print around the border are the words 'Thy Vows are upon me, O God'/ 'Lay hold on Eternal Life'/ 'Our help is in the name of the Lord'/ 'Fight the good fight of faith'. Central body of text surmounted by red cross with words 'Diocese of Perth/WESTERN AUSTRALIA' then in purple print and handwritten black ink 'Ruby Maude Wells'/Baptised on 12th April 1896/at Chittering / Confirmed on the 10th December 1901/at Wannamal/BY THE/ Right Rev. the Lord Bishop of Perth,/First Communion on 18th December 1910/at Wannamal/PRESENTED BY J. A. Howes/SIGNED C. O. R. Perth.
