Three small metal medallions mounted in circular recesses in a rectangular block of polished timber with bevelled edges with rectangular engraved silver coloured plastic? plaque adhered below with the words 'BINDOON NORTH SCHOOL - Ruby Maude WELLS/b. 1896 m. 1912 Colin A. A. MUNRO. d. 1974/8 of 12 children of Edward & Emily (nee Byrne)'
Each of the medallions sits loosely in a circular recess. They have identical inscriptions to obverse and reverse. Obverse shows a central raised carved image of a swan with the words 'EDUCATION DEPARTMENT/WESTERN AUSTRALIA' in a circle around the swan motif; the reverse of each medal has the inscription 'AWARDED TO/.../FOR PUNCTUAL/ATTENDANCE/EVERY SCHOOL DAY'
The medal for 1901 has the name Ruby Maude Wells engraved above the words 'FOR PUNCTUAL ATTENDANCE'. The two medals for 1903 and 1904 have the name 'Maude Wells' engraved in the same position.
