BURTON/10 AMHURST ST/FREMANTLE' handwritten in blue on front. 'MAUREEN DAPHNE BURTON/28/10/24' and 'FEMALE' handwritten in blue inside card. Table printed in black ink with dotted lines separating rows, with space for diseas, vaccine, dose, date, and signature. The first two lines are stamped in blue with 'POLIO POLIO VACCINE ?CC' and the dates '9-FEB 1959' and '24 MARCH 1959'. THe third line has '3rd vaccini 8th Dec 1959' handwritten in blue biro.
