Circular enamel badge with curved banner attached at bottom, with gold gilt finish raised rim. Circular top section with dark blue enamel border around outside edge with text 'CROATIA N. PERTH/ SOCCER CLUB', border encircles red and white checkered pattern. Red enamel curved banner attached to bottom of circular piece, with text 'LIFE MEMBER'. Reverse: gold gilt finish, text 'SHERIDAN/ PERTH' at top centre, clutch fixing with half pin attached and no clucth.
