The certificate is a single sheet of ivory coloured paper. It is hand written and printed. The hand written sections have a red underline. 'No 101/The Institution of Surveyors/ WESTERN AUSTRALIA / INCORPORATED' under this a printed Black Swan image. "By this Diploma Certifies that/ Bernard Walford Ridley/ is a Life-member -of the said Institution/ and fully entitled to all the prvileges granted/ in its Constitution/ As Witness our hands,for/ and on behalf of the Institution, this 7th day of August in the year of our Lord 1939." Signed the president and secretary. Photograph in the left hand corner of a youthful John Forrest.
The reverse stamped ink oval with' McKENZIE COLLECTION/1969 - 1984/ drawing of a 'hobby horse on two bike wheels'/ MUSEUM OF COLLECTION" on the inside of the oval.
