A rectangular metal plate with four nails at each corner holding it to a veneered wooden base. The stamp has the words 'THE HEALTH EDUCATION COUNCIL OF WESTERN AUSTRALIA' surrounding an image of a stork with a nappy in beak. Plate sitting off centre to base. Small torn piece of paper/tape adhered to bottom of base.
